Sources & methodology
Official facility records, with an independent local perspective.
What the directory covers
Our directory covers Medicare- and Medicaid-certified U.S. nursing homes using the CMS Certification Number as the facility identifier. It does not cover every assisted living community, hospital or home care provider.
Official sources
CMS Provider Information supplies identity, certified beds, ownership, ratings and reported staffing. Separate CMS datasets supply health and fire citations, survey summaries, penalties, ownership, MDS resident measures and claims-based outcomes. SNF Quality Reporting Program data adds rehabilitation measures; the FY 2026 Value-Based Purchasing file adds payment-program performance. Each section links its source.
Local comparisons
County and state averages are calculated from the imported provider snapshot. Facility benchmarks exclude the facility itself and ignore missing scores. Location overview averages include all homes in that location with reported scores. Averages are arithmetic means across facilities, not bed-weighted or resident-weighted. Sample sizes are shown for staffing and county ratings.
Staffing and weekend coverage
Nursing hours are reported hours per resident per day. Weekend difference equals (weekend hours ÷ all-days reported hours − 1) × 100. It is not a comparison with weekdays alone. Reported hours are not adjusted for resident acuity and do not establish individual shift coverage. Turnover is the CMS-reported percentage, not our estimate.
Ratings are not reviews
CMS star ratings are regulatory quality ratings. We do not display them as consumer reviews, combine them into an invented rating or promise a particular care outcome. CMS abuse-related flags indicate concerns reported by CMS and should be checked against current official information.
Dates, missing values and limitations
Source processing or publication dates are shown separately from individual measure periods. These are snapshots, not live availability. Blank, suppressed and footnoted values are not zero. Record counts describe the imported publication window; no records does not establish no historical events. VBP fiscal year and measurement periods differ. Payment multipliers are not prices for residents.
Choosing care
Use this information to prepare questions, not as medical advice or a substitute for a visit. Verify current staffing, services, insurance acceptance, costs and availability directly with the facility. The site is independent and is not CMS or Medicare.
