    Nursing home visit checklist

Bring your shortlist, the person’s care needs and questions tied to each facility’s record. Write down the answers and compare them after the visit.

    Before the visit

Confirm that the facility can support the required care, whether an opening is available and which payment sources it accepts. Read the dated CMS staffing and inspection records before you visit. Arrange accessibility or language support if needed.

    Care and staffing questions

Ask who develops and reviews the care plan, how medication questions are handled and who responds to urgent concerns at night. Discuss therapies, equipment, supervision, staff training and how absences are covered. Do not assume a listed category confirms the specific service required.

    Observe respectfully

Look at how staff interact with residents and respond to requests. Ask about cleanliness, infection prevention, meals, activities and visiting arrangements. Respect residents’ privacy; do not photograph or record people without appropriate permission.

    Ask about the inspection record

Discuss specific dated findings rather than general assurances. Ask what corrective actions were taken and how the home checks that changes last. A past citation alone does not tell you what is happening today, and a star score cannot replace the visit.

Get payment and contract details in writing

Request current rates, included services, extra charges, deposits, refund terms and discharge notice provisions. Ask what happens if care needs or coverage change. Confirm payer eligibility with the actual plan or state agency, not solely with the directory.

    After the visit

Compare notes with the person receiving care and the treating clinician where appropriate. Follow up on unanswered questions. Confirm services, availability and payment again before making a decision. You can print this guide for your visit or use Medicare’s official checklist below.
