Warren Hills Rehabilitation and Nursing Center strives to provide the highest quality care in a warm, homelike setting administered by a highly qualified interdisciplinary team. This team includes not only the resident’s physician, pharmacists, nurses, social workers, dietitian, activities coordinator and therapists, but the resident’s family also.

We are a 160 bed facility offering skilled and retirement levels of care, located in a beautiful rural setting.
Warren Hills offers physical, occupational and speech therapy for residents as ordered by a physician.
We encourage resident to participate in daily group activities including various games, exercise, non-denominational religious programs as well as providing in-room activities and engagement for those who are unable to attend in a group setting.
All meals are prepared by a trained dietary staff.
We encourage resident to dress in comfortable street attire since it enhances feelings of well being.
Barber and beauty shops are available for a charge. There is no charge for basic hair care.
Residents are able to make local phone calls, as well as receive phone calls at the nursing station. If a phone is needed in a residents room, the family must make arrangements with phone company.
Residents may send and receive mail Monday through Saturday.
Wheelchairs and walkers are provided for skilled nursing residents, if needed, for transport to and from various activities.
Residents may bring a television and cable hook-up is available for a fee.
Various sources of financial arrangements are available as applicable, including, Medicare, Medicaid, private insurance and private pay.
Daily rates are for room, board, and nursing care.
