Welcome to Salem Transitional Care. We are the new home for post-hospital rehabilitation and nursing services for Salem and Keizer residents. Set on five acres of beautiful land, our 53,000 square foot campus is home to 79 short-term skilled nursing beds, four specially designed bariatric suites, and modern amenities unique to our building. Our continuum of care is designed for fast recoveries and to return our patients to doing what they love most – living life to its fullest.
