Madison Health and Rehabilitation is a 60-bed Skilled Nursing Facility that offers both private and semi-private rooms. Our facility offers a wide range of amenities and services specializing in short-stay rehabilitation and long-stay care.

We are dedicated to improving the functional ability of each individual. We focus on the individual needs of each of our patients and partner with physicians, patients, and their families to achieve successful outcomes.

Our interdisciplinary team works together to improve patient outcomes, especially as it relates to preventative health and wellness programs, medical supervision and reducing the risk of a re-hospitalization. The facility’s rehabilitation gym is equipped with specialized therapeutic modalities and has therapists who are experts in their fields. Our patients have the resources and support to achieve their highest level of functional independence.
