Our 60-bed sub-acute rehabilitation center serves patients who have experienced an acute medical event (i.e. stroke, head trauma, illness) and need a "bridge" between a hospital stay and a return to home, usually lasting less than a month. This two-story center is connected to the existing Jefferson Health Care Center, with an enclosed courtyard between them so residents from both can spend quality time outside. There are both private and semi-private patient rooms and even patients in a shared room enjoy a sense of privacy, as the rooms are designed with separate bedroom space. Each floor has a kitchen and dining area open to both patients and visitors. These kitchen areas are arranged so they may also serve as a training area for patients receiving occupational therapy. Physical, speech and occupational therapy are provided in the second-floor gym. The landscape plan includes a healing garden in the courtyard and the lobby has a modern waterfall element.

The Jefferson Health Care Center has four-star ratings in the category of “Quality Measures,” and “Overall Staffing” and a five-star rating in “Nursing Staffing” from Medicare.gov/Nursing Home Compare.
