Concord Health Care, Inc., operates four licensed nursing homes in the state of Ohio, all of which are Medicare and Medicaid certified. Concord provides skilled and intermediate levels of health care for short-term and long-term residents. Respite services and rehabilitation services are offered at all Concord Care Centers, including physical, occupational, and speech therapy. Resident transportation is also available. Two locations dedicate a portion of their buildings to serving individuals with behavioral challenges. These units are known as The Vista at Concord Care Centers.

Concord Care Centers of Hartford and Cortland are located in Trumbull County. These fifty-bed homes offer quiet, picturesque, country settings and are just minutes away from the surrounding cities of Warren, Cortland, Howland, Girard, Hubbard, and Youngstown, and minutes from western Pennsylvania. Secured environments are available at both facilities.

Concord Care Center of Sandusky is located in Erie County. Also known as Concord Care and Rehabilitation Center, our Sandusky care center is a fifty-bed facility, and is located in a popular summer vacation area near the shores of Lake Erie. It offers a quiet secluded setting that is just one mile from route 250, where shopping, dining, and activities are abundant. A leading edge rehabilitation center was opened in September, 2006 with specially designed programs and equipment to meet the individual's therapeutic needs.

Concord Care Center of Toledo is located in Lucas County. It is part of a residential area in southwestern Toledo, near Maumee and Perrysburg, and just minutes from most of Toledo's major attractions. It is an eighty-four-bed facility with newly remodeled resident rooms, eight of which offer private accommodations. Toledo also houses an eighteen bed Vista unit.
