Carbondale Nursing and Rehabilitation Center is dually certified in Medicare and Medical Assistance. Our nursing department is staffed by registered nurses, licensed practical nurses and certified nursing assistants who provide 24-hour care. All care and services are under the supervision of a Medical Director. Our dietary department is under the direction of a Registered Dietitian. We ensure that the nutritional needs of each patient and resident are met and provide home cooked meals in a relaxed dining room atmosphere for our patients and residents. Carbondale Nursing and Rehabilitation Center offers the following services: ShortStay Rehabilitation, LongTerm Care, Speech, Occupational and Physical Therapies, Recreational Programs, Hospice Services, Respite Care, Wound Care Management and Pain Management.
