Short-term Rehabilitation

End-of-life Care
We recognize that loss and anxiety accompany the end of life and are shared by both individuals with terminal illness and their loved ones. At Autumnwood, we provide medical, emotional and spiritual assistance to people facing the end of life, in a comfortable and familiar place, often in partnership with hospice programs. Each member of our staff is specially trained to provide compassionate, attentive end-of-life care.

Respite Program
