San Francisco Health Care and Rehab (previously known as Nonprofit Skilled Nursing Facility Grove Street Extended Care And Living Center) is a Skilled Nursing Facility with 168 Medicare and Medicaid certified beds located east of Golden Gate Park in San Francisco.

Our diverse and multilingual (i.e. Cantonese, Mandarin, Tagalog, Spanish etc.) team of staff is here to care for those who require comprehensive nursing care and rehabilitation services.

Services provided by San Francisco Health Care include services of nurses, nursing aides and assistants; physical, occupational and speech therapists; social workers and recreational assistants; and room and board.
