Welcome to Queens Boulevard Extended Care Facility. We are a modern, state-of-the-art Nursing Facility with a 280-bed capacity which opened in 1995. Our facility is conveniently located in Western Queens and is easily accessible to all major highways, close to subways and bus routes.

Our Mission is "to provide the highest quality health care in a caring environment", and to meet the needs of the diverse communities that we serve. We offer high quality skilled nursing and medical care as well as comprehensive services in physical, occupational and speech therapy. We specialize in sub-acute short-term rehabilitation. Recreational programs are scheduled everyday and special events are designed to meet the interest of each individual. Nutritional services are carefully monitored by our dietitians and special consideration is given to meet each resident's nutritional needs.

As part of our commitment to quality care and services, we have an active Quality Improvement Committee and have received accreditation from the Joint Commission on Accreditation of Health Care Facilities with a perfect score of 100!
