Porthaven Health Care Center is a skilled nursing and rehabilitation center that serves the northeast Portland neighborhoods of Rose City, Beaumont, Alameda, Hollywood and Irvington. We work hard to make your stay here enjoyable and vibrant.

Our goal is to assist you with the transition between hospital and home. Our rehabilitation programs are a wonderful solution for individuals who are stable and no longer require hospitalization, but do need intensive rehabilitation and recovery. Our patients find that our therapy gym is ideal for improving range of motion, and the many other amenities make it the perfect place for occupational therapy, when individuals need help patients regaining full functionality.

For residents who need assistance with routine personal care activities, our center also offers health services coordination by licensed nurses, 24-hours a day. Porthaven Health Care Center is also a contracted provider for Medicaid, Medicare and several managed care plans.
