Our modern facility opened its doors in November of 2012, with every need of our residents in mind. Autumn Wood was built for residents to enjoy life in an environment that feels as comfortable as home. When you walk into Autumn Wood you won’t see a nurse’s station even though we provide the same level of medical care found in a traditional nursing home. It feels like home. Our residents enjoy security in neighborhood style living. What does that mean? Our facility consists of 5 individual neighborhoods, each complete with private dining rooms, common area rooms and a spa area. Within the neighborhood are 12 individual rooms, each with its own private bathroom, for a total of 60 rooms. Residents can get to know their neighbors, dine with the same folks every day and even enjoy a private patio area complete with gardens and bird watching. This unique design contributes to a greater quality of life with highly individualized care without the institutional feel of a traditional nursing home.

Familiarity is important to those living with Alzheimer’s or other dementia. We want to minimize any anxiety or worry that may trigger any agitation. Our staff is highly trained to manage all kinds of behavior and situations. Staff are assigned to the neighborhoods with a minimum of 80% consistency to help your loved one feel more comfortable as they will live, dine and experience community with the same staff every day. Autumn Wood is a skilled nursing facility and in addition to long-term dementia care, we also provide short-term rehabilitation. Rehabilitation may be needed after a stay in the hospital due to surgery or other illness.
