Grafton County Nursing Home is a 135 bed skilled nursing facility owned and operated by Grafton County. Our mission is to ensure each resident experiences the highest quality of care and quality of life in a warm, welcoming, homelike setting. We accomplish this goal by caring for our residents as we would for one of our own family members, because we view each of our residents as a member of our extended family.

Our home offers private and semi-private rooms. Under the direction of our Director of Nursing, we offer twenty-four hour nursing coverage. We also have a part-time physician and full-time advanced practice registered nurse (APRN) on staff to ensure each resident’s care needs are met any time of day. Our service offerings also include in-house rehabilitative programs; meals and baked goods made fresh daily; housekeeping and laundry services; beautician services; social work; business office assistance with financial matters; and an activities program which provides a wide array of programs designed to meet each resident’s particular interests.
